You can access your medical records through the NHS App.

If you would like to request historic information from your medical records - called a Subject Access Request (SAR) - please download the request form (DOCX, 22KB). Although submission of this form is not mandatory, it will allow us to make sure your request is quickly directed to the correct team for processing.  

Please be assured that we have always taken our patients data privacy very seriously, and will continue to do so.
